Edwardsburg Rocket Cheer Participant Agreement

Emergency Medical Authorization

In the event of an injury or medical emergency involving my child, I authorize Edwardsburg Rocket Cheer representatives, coaches, or volunteers to obtain necessary medical attention for my child. I grant permission for a licensed physician, athletic trainer, emergency medical technician, or other qualified medical personnel to provide medical care and/or transport my child to an appropriate medical facility if deemed necessary.

I understand that every reasonable effort will be made to contact a parent or guardian prior to treatment. However, if a parent or guardian cannot be reached in an emergency, I authorize medical personnel to provide appropriate care as deemed necessary for my child’s health and safety.

I also authorize the release of any necessary medical information to healthcare providers for the purposes of treatment, referral, billing, or insurance purposes.

Liability Waiver and Assumption of Risk

I understand that participation in cheerleading activities involves inherent risks of injury, including but not limited to falls, physical contact with other participants, and physical exertion.

By registering my child for Edwardsburg Rocket Cheer, I voluntarily assume all risks associated with participation. On behalf of myself and my child, I release, waive, and hold harmless Edwardsburg Rocket Cheer, its board members, coaches, volunteers, and representatives from any and all claims, liabilities, damages, or expenses arising from injury, accident, or participation in any Edwardsburg Rocket Cheer practices, events, travel, or related activities.

I understand that Edwardsburg Rocket Cheer does not provide medical or accident insurance for participants and that it is my responsibility to ensure my child is covered by personal medical insurance.

I acknowledge that cheerleading is a physical activity that carries inherent risks that cannot be completely eliminated, even with proper supervision, training, and safety precautions. I agree that my child will follow all safety instructions, rules, and decisions made by Edwardsburg Rocket Cheer coaches, volunteers, and program leadership. I understand that failure to follow program rules or safety expectations may result in removal from participation.

I understand that Edwardsburg Rocket Cheer may occasionally organize or promote optional team activities (such as fun nights with Tug-Of-War and other carnival like games, go-karts, skating, bowling, or similar outings) held at third-party locations. I acknowledge that these venues are not owned or controlled by Edwardsburg Rocket Cheer, and I release and hold harmless Edwardsburg Rocket Cheer, its board members, coaches, and volunteers from any liability for injuries or incidents that may occur during these activities.

Concussion Awareness and Return-to-Play Acknowledgement

I acknowledge that participation in cheerleading and athletic activities may involve the risk of concussion or other head injuries.

I understand the importance of recognizing the signs and symptoms of a concussion, which may include headache, dizziness, confusion, nausea, balance problems, sensitivity to light or noise, or changes in behavior.

If my child is suspected of sustaining a concussion or head injury during any Edwardsburg Rocket Cheer activity, they will be removed from participation and will not be permitted to return to practices, games, or related activities until evaluated and cleared by an appropriate licensed healthcare professional.

I agree to follow all return-to-play guidelines and provide written medical clearance before my child resumes participation.

Photo / Social Media Release

I grant permission for Edwardsburg Rocket Cheer to photograph or record my child during practices, games, events, or activities. I understand that these images or recordings may be used for promotional, educational, or informational purposes, including but not limited to use on social media platforms, websites, printed materials, and other promotional materials related to Edwardsburg Rocket Cheer.

No personal identifying information other than first names may be used unless additional permission is granted. I understand that no compensation will be provided for the use of such images or recordings.

Refund Policy

All registration fees for Edwardsburg Rocket Cheer are non-refundable.

The only exception applies to 6th grade participants who earn a position on the Edwardsburg Middle School cheer team. Upon confirmation of team placement, a partial refund may be issued.

Parent / Guardian Acknowledgement

I certify that I am the parent or legal guardian of the participant listed on this registration. By signing below, I acknowledge that I have read, understand, and agree to all terms outlined in the Edwardsburg Rocket Cheer Participant Agreement.

I understand that participation in Edwardsburg Rocket Cheer is voluntary and that my child and I agree to abide by the policies and safety guidelines established by the organization.
